Add integration test infrastructure

- Create backend integration test project with xUnit
- Add required packages: TestHost, Mvc.Testing, EF InMemory, FluentAssertions
- Add project reference to backend API
- Create IntegrationTestBase class with WebApplicationFactory setup
- Install Supertest for HTTP integration testing
